SUMMARY
Agreeing allows YoungBlood (Youngblood Services LTD) and their successors, hereafter THE COMPANY, the photographer, teaching staff and other participants who do not opt out of this agreement to use pictures and recordings of you taken during this workshop for the purposes of promotion, advertising, non-profit making documentary, education and PR.
If you decide to opt out of this release usage will then be limited only to educational purposes, which does not require a release.
TERMS
- I agree to allow THE COMPANY and individual TEACHING staff members and participants of this workshop perpetual use of my image and recordings of myself in classes, rehearsal, and performance, taken or recorded during workshop activities.
- Media includes audio, video, digital or analogue recordings as well as photographic media.
- Use by THE COMPANY, teaching staff and workshop participants will be strictly limited to the following purposes: education, archives, promotion, advertising, and public relations. I DO NOT permit THE COMPANY, or any other party TO SELL OR TRANSFER FOR PROFIT any images or media from this event featuring me, without further agreement between THE COMPANY, the photographer (or videographer), and myself.
- For the use of video material online, the COMPANY and Choreographer must be credited.
- I understand that my image may appear in media and showreel material that may be sold through other services to other members of the workshop. That particular image or recording is available for the following uses only: archives, personal promotion, personal advertising, non-profit making documentary and individual public relations.
- I do understand that some material from this workshop may be provided to the staff or participants for a fee.
RISK ACKNOWLEDGEMENT
By agreeing to these terms, participants and staff understand that participation in performance combat involves the risk of serious injury or death.
HEALTH ACKNOWLEDGEMENT
Participants agree to inform the instructor of their course at the earliest opportunity if any disease, condition or previous injury applies to you. We are concerned only about conditions and injuries that may make you more liable to injury, more liable to incur others, or may affect how you train or interact. Examples include.
- Haemophilia
- Previous joint, back, neck or head injury or surgery (please provide detail)
- Migraines
- Heart problems
- Epilepsy
- Diabetes
- Respiratory problems e.g. Asthma
- Nervous Disorders or Mental Health Issues
- If you on any medication that will affect your abilities
- Hay fever/ allergies (ie. to horses)
- Any other disease, condition or previous injury which may may affect your ability to work safely or to interact with others.
